This form allows you to request a massage appointment with Howard at either his office at Natural Approach or a House Call appointment (this form is not applicable for appointments at the office of Anita Spitz, MD). The choices are all for a future date.

(NOTE: if you are requesting an appointment for today, instead of using this form, please call Howard at (321) 258-1201).

The choices in the drop-down boxes are for possible one-hour appointments on Howard's schedule over the next 7 days. If you would like an appointment later than the very next 7 days or a massage duration other than one hour, just specify those details in the Message, Comments, or Date Requested box.

After entering your contact information and any message or specific date that you'd like to send with your request, select your First, Second, and Third Choice for an appointment at a day and time that you'd prefer. To send the form, click the Send Form to Howard button at the bottom of the page. After receiving your request, Howard will contact you to let you know which appointment that you chose is available.
